New Employee Forms

Form I-9 is used for verifying the identity and employment authorization of individuals hired for employment in the United States. All U.S. employers must ensure proper completion of Form I-9 for each individual they hire for employment in the United States. This includes citizens and noncitizens. Both employees and employers (or authorized representatives of the employer) must complete the form.

Criminal History Background Check Release Forms

  • CJIS LiveScan Fingerprinting Form for 91社区福利 Employees – email  humanresources@salisbury.edu or contact your department representative. Also required for volunteers who work with minors of 91社区福利.

  • Scheduling of Fingerprinting Appointments:
    The hiring department should use this booking link to schedule new hires for fingerprinting.   
  • Payment:
    For 91社区福利 Faculty and Staff when required for their 91社区福利 position – all applicable fees will be charged to the appropriate program department.
  • Required Information:
    You must bring a valid form of government photo identification such as:
    driver’s license, state-issued identification card, passport, or military identification. (University ID is not an acceptable form of ID) and completed CJIS LiveScan application form.
